Health declaration and SSI
Before anyone goes in the water there are a few documents.
Here is a short explanation, with links to the village's original documents and forms.
The rules themselves have not been rewritten.
Health declaration
The purpose of the medical declaration of SSI is to find out whether you need to be examined by a doctor before you take part in a diving course.
A positive answer to a question does not automatically disqualify you from diving.
A positive answer means that there is a pre-existing condition that may affect your safety while diving and that you must consult the doctor.
If there are health problems, or if you are over age 45, you must provide approval from a diving doctor only!
It is important to read and fill in the health declaration and to make sure there is no past or present history of the various conditions
- Health declaration for courses (PDF, in Hebrew)
- Health declaration for the intro dive (image, in Hebrew)
- Read and fill in the health declaration in the app.
It is important to check, and to come to the course after you have completed the health declaration in the app, correctly and in full - If one of the answers is “yes”, check the second page and get the diving doctor's approval
Filling in the medical declaration
- Go into the SSI app.
If you have not registered yet, see the detailed instructions here. - Tap the settings icon
- Choose "Personal details"
- Tap "Training records and documents"
- Scroll to the medical declaration, tap it to reveal the buttons, and tap again on "Fill in online form"
